Transplantation is considered as a „new chance for life“ for patients in end stage of a solid organ failure. However, it involves a lot of
restrictions in patient common life. Health care of organ transplant recipients entails regular medical examinations, regime and diet
recommendations and special rules in immunosupressive drug administration. These are necessary for solid organ graft acceptance
and its right function. There are more drugs used to lower risk of transplant complications or to treat other patient comorbidities in organ
transplant recipient medication. Patient with polypragmasy has to be explained rules how to take his medication to avoid potential
drug-drug or drug-food interactions. It is a role of pharmacist to provide right drug counselling and to give patient important diet and
regime recommendations.
