Individual counseling in community pharmacy - case report of geriatric patient

Individual consultations in pharmacy provide a way to identify and solve drug-related problems (DRP). The aim of this geriatric patient

case report is to present continuity of the management of DRP including difficulties accompanying the care providing. Several

types of DRP were identified. Pharmacist`s interventions were accepted by both patient and doctors. Individual consultation helped

to increase patient adherence to treatment and optimize the pharmacotherapy.
