Antiepileptics and their clinical use in epileptology

Antiepileptics are drugs used to treat or suppress epileptic seizures. Currently, over twenty antiepileptic drugs are registered in the Czech Republic. Some of them are frequently used in all types of epileptic seizures while others are indicated in limited circumstances due to selection efficiency or poor tolerability. The widely used classic antiepileptics include carbamazepine and valproates and the widely used novel ones include lamotrigine, levetiracetam and topiramate.

The article builds on previously published issue about diseases of the oral mucosa. It is focused on the possibility of specific pharmacotherapeutic intervention to mucosal diseases by local therapy with particular emphasis on local antiseptics, non-steroidal anti-inflammatory drugs, corticosteroids, topical antibiotics, chemotherapeutics, and antifungals.

Hearing loss is one of the most common diseases in the population. The treatment of hearing loss depends on the site of origin of the disease in the auditory pathway, the causative factor and its duration and intensity. The prognosis of sudden hearing loss is the better, the sooner (within hours and days) intensive treatment is initiated. Individual treatment methods are mentioned. The results of all types of treatment for sudden sensorineural hearing loss are similar: 60–80 % of patients are cured, 20–30 % improve and 10 % to 25 % do not respond to treatment. Onychomycosis

Onychomycosis is a mycotic infection of a nail apparatus that consists of a nailplate, nail matrix and a nailbed. The disease itself is not life threatening but could cause in patients and especially immunocompromised subjects serious complications. In some untreated forms it could cause significant pain that leads to psychological and social problems and decrease of patients’ quality of life.

Immunosuppression after kidney transplantation

Successful kidney transplantation depends on the quality of the donor organ, surgical procedure and immunosuppressive regimen. Immediately before and shortly after transplantation, induction immunosuppression is used, based on T lymphocyte depletion or blockade of function. In patients at risk of rejection, antithymocyte globulin is used while, in patients at low risk, monoclonal antibody basiliximab against interleukin-2 receptor is used. Long-term immunosuppression mostly consists of a combination of calcineurin inhibitor (tacrolimus or cyclosporin A) together with mycophenolate mofetil, an inhibitor of purine synthesis, and steroids. Atopic dermatitis (AD) is one of the most common skin conditions affecting mainly children but also adults. The etiopathogenesis of the disease, its incidence and course and typical as well as less common clinical manifestations are briefly introduced. Complications of the course of AD are briefly mentioned. Major attention is paid to external treatment and preventive care of atopic skin, particularly in childhood. Risk and aggravating factors and options of systemic treatment are also mentioned.
