In recent years, new active substances, dosage forms of previously known agents as well as new combinations of drugs have emerged in the field of pharmacotherapy of pain. Among active substances, a new molecule in the class of opioids has been encountered after more than 30 years. The new substance is tapentadol, a μ-opioid agonist capable of norepinephrine reuptake inhibition in the CNS. Vitamin D is one of the lipophil vitamins, which one is necessary for correct structure and function of the skeleton. The lowered saturation of vitamin D plays an important role in immunity disorders, cardiovascular diseases and oncological too. Reduced level of vitamin D is an important risk factor of fall and fracture at older patient. Colitis due to Clostridium difficile is within the past decade cause of epidemics worldwide and raising prevalence is being reported. Geriatric patients with numerous comorbidities who were repeteadly hospitalized and recently treated with broad-spectrum antibiotics are mostly affected. Treatment with aminopenicillins, cefalosporines, fluorochinolones and lincosamides is therefore of great concern. The disease may be treated either with vancomycin (orally), metronidazole or fidaxomicine. Fecal microbiota transplantation may be used in recurrent disease. The authors presentbasic informations about this severe condition and its treatment.

Bipolar disorder (BD) is a severe, mainly reccurent, multifactorial mental illness. Ĺithium with its antimanic, antidepressive, antisuicidal, prophylactic and neuroprotective properties has remained a major treatment of BD. In acute mania lithium, valproate, and several atypical antipsychotics in the monotherapy or combination with mood stabilizers are first-line treatments. For the management of bipolar depression, lithium, lamotrigine, and quetiapine monotherapy, as well as olanzapine plus selective serotonin reuptake inhibitor (SSRI), and lithium or valproate plus SSRI/bupropion remain first-line options. The aim of the article is an effort to capture the historical development of insulin production in Czechoslovakia. Information has been obtained mainly from two czech journals: „Časopis lékařů českých“ (published since 1862) and „Praktický lékař“ (published since 1921). At first, imported insulin appeared on the market in Czechoslovakia; insulin injections or substances from which insulin was prepared. Production of insulin was dealt with fivecompanies in years 1923- 1945, then the insulin was produced just by one company „Léčiva“.

Individual consultations in pharmacy provide a way to identify and solve drug-related problems (DRP). The aim of this geriatric patient case report is to present continuity of the management of DRP including difficulties accompanying the care providing. Several types of DRP were identified. Pharmacist`s interventions were accepted by both patient and doctors. Individual consultation helped to increase patient adherence to treatment and optimize the pharmacotherapy.
