The elderly are the largest consumers of prescribed medications. Prescribing for the elderly is complicated by a variety of factors.The presence of multiple chronic conditions and multiple medications leads to potentially complex drug-drug and drug-diseaseinteractions. Several tools and algorithms are available to aid clinicians in prescribing of adequate treatment. Good clinical assessment,an understanding of the patient´s overall condition and individual treatment goals remain pivotal to provide high qualitycare and to decrease adverse drug reactions occurence.

A large proportion of patients treated for depression also suffer from somatic diseases. It is often difficult to choose an appropriateantidepressant drug while preventing deterioration of the patient’s physical condition and avoiding drug interactions. Agomelatineis a highly effective antidepressant with very low rates of adverse effects and drug interactions. It appears to be a suitableoption in patients with cardiovascular, neurological, and metabolic disorders.

Lapacho (Tabebuia impetiginosa) is a tropical evergreen tree originating from South America. Its bark is used in traditional medicineespecially for cancer, infections a immunity disorders, such as allergy, revmatism, and psoriasis. The main active compoundsof lapacho are quinones lapachol and β-lapachon and recent studies confirm its therapeutic effects. The most promising is theiranti-tumor and immunomodulatory activity. However, clinical trials are still lacking.

The growing incidence of allergy symptoms and allergic diagnosis increases the demand for safe self-medication. Among over-thecounterdrugs we include symptomatically acting active ingredients from a group of antihistamines for systemic and topical therapyand corticosteroid for topical application. Self-medication is focused mainly on the management of less severe acute allergic symptoms.

The surveillance tables provide examples and composition of OTCdermocosmetics and medicines that are available in pharmacies. The examples given dermocosmetics brands were selectedbased on their market position, long-term experience of the companies in the development of the products, collaboration withdermatologists and based on the positive response to the patient’s side.

Smoking and a tobacco addiction is a chronic disease being among the risk factors for the emergence of a range of diseases.Smokers are a threatened group even for postoperative complications and for the process of the operation itself. Passive smokingis also underestimated. There are more than 2 million people addicted to tobacco in the Czech Republic, 18 000 people out ofwhich annually die. Half of smokers die 15 years earlier than if they would not smoke.
