Pharmacotherapy of inflammatory bowel diseases

prof. MUDr. Milan Lukáš CSc

Prakt. lékáren. 2009; 5(4): 164-167

Aminosalicylates, corticosteroids, immunosuppressants, and recently also biological therapy are the mainstays of pharmacotherapy of inflammatory bowel diseases (IBD). Aminosalicylates (sulphasalazine, mesalazine) are essential drugs for both aggressive and maintenance treatment of patients with ulcerative colitis. They are administered orally, in limited disease they may also be administered locally in the form of suppository or rectal enema. Corticosteroids with systemic effect have a strong anti-inflammatory effect and are used in oral or parenteral therapy in patients with a severe course of intestinal inflammations. The treatment with corticosteroids...

prof. MUDr. Jan Štěpán DrSc

Prakt. lékáren. 2009; 5(4): 168-171

This review examines the clinical data available to date on pharmacological properties of calcium and vitmain D supplements, hormone therapy and tibolone, selective estrogen receptors modulators, and strontium ranelate in prevention in treatment of osteoporosis, their effects and contraindications.

Overview of opioid analgesics

MUDr. Jan Lejčko

Prakt. lékáren. 2009; 5(4): 172-175

Opioids are an irreplaceable type of analgesics intended for management of intense acute pain, tumor-related pain, and, in numerous cases, chronic non-tumor-related pain. Their adequate use has been hindered by the phenomenon of opiophobia. In the Czech Republic, there is currently a sufficient selection opioids allowing for the necessary individualization of pain treatment. Professionally guided treatment with opioids results in improved pain management as well as improved functional status and quality of life.

Pharmacotherapy of Alzheimer's disease

doc. MUDr. Roman Jirák CSc

Prakt. lékáren. 2009; 5(4): 176-178

In this article there are got through contemporary possibilities of pharmacotherapy of Alzheimer´s disease from the evidence-based medicine point of view. Only 2 approaches are evidence-based at the present time: cholinesterase inhibitors and memantine. There are hereafter discussed the possibilities of accessory unambiguously non-evidence based medication, possibilities of symptomatic therapy of behavioural and psychological symptoms of dementia and the possibilities of use of over the counter preparations as the auxiliary agents of therapy.

Proper inhalation technique of drugs and errors

prof. MUDr. Vladimír Vondra DrSc

Prakt. lékáren. 2009; 5(4): 179-181

The choice of inhalation device and correct inhalation technique is fundamental for successful treatment of obstructive pulmonary diseases. There were described contemporary devices – meter dose inhalers, dry powder inhalers and nebulisers used in Czech Republic. The improvement of treatment effectiveness could be reached with non fixed or fixed spacers (e.g. JET system). The education of inhalation technique is in the Czech Republic for several years mostly realized through allergology and pneumology organizations. Nevertheless, all patients, children as well as adults, do some significant mistakes in inhalations (38 % and 40 %). Czech results...

Sitagliptin - a representative of incretin treatment for type 2 diabetes

prof. MUDr. Jaroslav Rybka DrSc

Prakt. lékáren. 2009; 5(4): 159-162

Incretins are hormones produced by gut cells following a postprandial stimulus. They have a polypeptide structure a very short plasma half-life. Due to their polypeptide structure however, incretins represented specifically by the glucagon-like peptid 1 – GLP-1 (glucagonlike peptid 1) accounting for over 60 % of the „incretin effect“ and by the glucose-dependent polypeptide GIP (glucose dependent insulinotropic polypeptide) have a very short half-life. Therefore, the therapeutic potential of active forms of incretins (GLP-1 and GIP) could not be used for the managment of type 2 diabetes. Sitagliptin belongs to DPP-4 inhibitors,...

Prakt. lékáren. 2009; 5(4): 189-193

Submited article deals with therapeutic potential of cannabis metabolites. The cannabinoids as well as other bioactive secondary metabolites are introduced. Medicinal preparations based on the research of cannabinoids and human endocannabinoid system with dosage in particular indications are presented. In the following the crude cannabis drug is characterized. The Czech Republic legal provision concerning cannabis usage in pharmacotherapy is presented. The psychoactive abused potential of cannabis is briefly mentioned, especially with the emphasis on possible risks and the development of dependence.

Protection before the ticks and bugs

Mgr. Lenka Čechová

Prakt. lékáren. 2009; 5(4): 184-189

We can meet many kinds of Arthropods anywhere. The wasps, the hornets, the horseflies, the flies, the fleas, the mites, the ticks etc. can be found in our surrounding. We don´t need to go to the tropic areas, such as Africa, Asia or Amazonia. Also in our mild climatic conditions we can be endangered by serious infections transmitted by the Arthropods or we can suffer from various allergies caused by the mites or biting from insects. That is the reason why to use effective repellents.

European Court of Justice - ownership and operation of pharmacies can be restricted to pharmacists alone

PharmDr. Martin Dočkal

Prakt. lékáren. 2009; 5(4): 200-201

Italian and German legislation laying down such a rule is justified by the objective of ensuring that the provision of medicinal products to the public is reliable and of good quality. Today, the European Court of Justice (ECJ) has brought to a close two sets of proceedings relating to the system of pharmacy ownership (Commission v Italy and Apothekerkammer des Saarlandes and Others). With this decision, the ECJ followed the recommendation of Advocate General Yves Bot who had also voted in favour of the ban on outside ownership in his opinion on 16 December 2008.

Where errors are made when prescribing antibiotics

doc. MUDr. Vilma Marešová CSc

Prakt. lékáren. 2009; 5(4): 195-196

Antibiotic policy plays an important role in maintaining antibiotic efficacy. The amount of antibiotics consumed as well as the quality of choice of antibiotics concerning their properties, pharmacokinetics and pharmacodynamics, contribute to developing resistance. Thus, the strategy of antibiotic therapy involves correct indication, dosage, and duration of treatment. As the largest amounts of antibiotics are consumed with respiratory infections, the majority of incorrect indications occur with them.
