The irreplaceable role of vitamin D in prevention and treatment of osteoporosis

prof.MUDr.Sylvie Dusilová Sulková, DrSc., MBA

Prakt. lékáren. 2011; 7(2): 58-61

Osteoporosis and its consequences represent a major clinical and socio-economic problem. A low calcium intake and particularly vitamin D deficiency are the crucial factors resulting in or contributing to osteoporosis; supplementation with vitamin D is an essential condition for both prevention and treatment. Calcidiol concentrations below 50 nmol/l (= 20 ng/ml) are clearly unfavourable for bone metabolism; some authors extend this limit up to 70 nmol/l. Vitamin D3 (cholecalciferol) at 700–800 IU daily is the preferred type of vitamin D supplementation. Attention also has to paid to the minerals essential for bone tissue (daily calcium...

Prevention of recurrent urinary tract infections in practice

MUDr.Ladislava Lyerová, CSc., Mgr.Jana Gregorová

Prakt. lékáren. 2011; 7(2): 68-70

Recurrent urinary tract infections (UTIs) are a most frequent clinical problem affecting particularly women. Long-term use of antibiotic prophylaxis, although effective, has its limitations making search for alterative treatments an option. Our recommendation for the prevention of recurrent UTIs is to use herbal remedies containing cranberry products, D-mannose or nasturtium. Also effective is improvement of microflora using both oral and vaginal probiotics. However, the most important role in the prevention of these infections is played by immunotherapy represented primarily by oral vaccines (Uro-Vaxom, Urivac). The efficacy of this product...

Ustekinumab - a new option of psoriasis therapy

MUDr.Veronika Slonková

Prakt. lékáren. 2011; 7(2): 55-57

Psoriasis is a chronic inflammatory disease that can be treated with biologics. Ustekinumab is a new group of biologics called p40 blockers. Ustekinumab is a fully human monoclonal antibody that binds to the protein subunit p40 of the interleukins IL-12 and IL-23. This results in the blockade of the activation of T-lymphocytes that prevents their differentiation to Th1 and Th17 cells, with subsequent blocking of secretion of proinflammatory mediators (TNF-alfa, IFN-gamma, IL-17, IL-22). Ustekinumab is administered in subcutaneous injections, 45 mg for dose (or 90 mg for dose in patients heavier than 100 kg), at weeks 0 and 4 and than once every...

Digestion and resorption

doc.MUDr.Zdeněk Wilhelm, CSc.

Prakt. lékáren. 2011; 7(2): 71-75

Digestion and resorption play an essential role in the body’s metabolism. Disorders involving one or both of the functions mentioned above significantly affect the organism as a whole. In the case of disease, they codetermine the further course of the disease both in terms of providing the total energy needs and in terms of the optimal composition of the substrates ingested in a particular situation. Vulvovaginal candidiasis, pharmacotherapy and prevention

PharmDr.Petr Horák

Prakt. lékáren. 2011; 7(2): 80-84

Vulvovaginal candidiasis is one of the most common fungal diseases. Approximately 75 % of women will have experienced at least one episode of this disease during their life. The sporadic form of vulvovaginal candidiasis can be treated very successfully with topical or systemic azole antifungals in short or unit-dose regimens. Special care has to be devoted to therapy of pregnant women and complicated and recurrent forms of the disease. Self-medication is often involved in the therapy of vulvovaginal candidiasis; correct consultation from the pharmacist plays an important role then.
